What to Submit
Your application should be in two parts:
Part I: Grant Request Cover Letter OR Application Form
We invite you to submit either a grant request cover letter, OR fill out the Application Form provided. If you choose to write a grant request cover letter, please be concise (1-2 pages) and be sure to include:
- Your specific request (Your request should be clear, well thought out, and have realistic goals);
- Appropriate back up material to substantiate your request, i.e. budget, schedule, proposed outcome, etc.;
- If we are able to fund your request, indicate the specific geographic reach it would have, i.e. region, county, town; and
- Material that covers your agency's historical background.
You may use the Application Form provided as a guide for the information we are interested in receiving.
Part II: Grant Request Attachments
All agencies must submit current data that includes the following:
- Copy of your organization's IRS 501(c)(3) notification letter;
- Current list of board members;
- Most recent audited financial statement;
- Current IRS Form 990, if available;
- List of top 3-5 current business contributors and foundation grants including the level of support; and
- Informational brochure, if available.
If any of the above information is not included, and there is no explanation as to why, your proposal will not be submitted to our Board for review.
